Sorry for trouble, looks like I found an issue, It was Russian localization that added some custom CSS and even PHP files to adjust WP admin looks. Looks like this stuff was causing a trouble.

Dean, thank you for your great plugin, I’m really enjoying using it! However, I have some weird issue with it on my WordPress installation with Russian language enabled (define(‘WPLANG’, ‘ru_RU’);).
